COVID-19 SSA CHECKLIST

ALL PLAYERS.COACHES MUST TAKE THE TIME TO COMPLETE THE CHECKLIST BELOW FOR EVERY TIME YOU COME TO TRAIN AND/OR PLAY.

• Standardized Screening Assessment (SSA). Player/Staff needs to answer ALL questions. If all answers are NO, Player/staff will be allowed to come to the facility. If any answers YES, Player/staff will not come to the facility, and will contact their healthcare provider.

• Documented temperature upon entrance to facility
